Admission Requirements
- A three-day acute care hospital stay within 30 days prior to admittance in the Transitional Care program.
- A daily need for skilled care (nursing care seven days a week, physical therapy, or other rehabilitation five or more times a week).
- A skilled need that is related to a recent inpatient hospital stay.
Insurance Coverage
- Enrolled in Medicare Part A and Part B
Our team will work with other insurances on eligibility for benefit.
