Transitional Care (Skilled Care)

Transitional care, also known as swing bed or skilled care, is a Medicare benefit offering you or your loved one a comprehensive, skilled level program following an “Acute Care Inpatient” hospital stay (minimum stay of three consecutive midnights within a 30 day period).

As part of our transitional care program, you will be able to focus on your rehabilitation after an acute care stay. Our program gives you an opportunity to work with a multidisciplinary team to increase your strength and regain your independence.

Is transitional care for you?
Patients who benefit from an extended stay in our program include:

  • Individuals in need of rehabilitation or therapy after an orthopedic surgery, such as knee, hip, or other joint replacement.
  • Individuals recovering from a stroke, but do not qualify for admission to an inpatient rehab center.
  • Individuals needing to regain strength and mobility after an illness, injury, or surgery.
  • Individuals needing skilled nursing support, education and management of a chronic condition, such as diabetes and congestive heart failure prior to returning home from an acute hospital stay.
